School Documents For Volunteering
​The following documentation will need to be submitted via this secure email – ossrecordkeeper@pacificasd.org OR
in the blue document box above the PTO Safe located at the beginning of the kindergarten hallway.

Classroom and General School Volunteers
  • Volunteer Application Form
  • Current Proof of Negative TB Test
    • You can submit current proof of TB via any one of the following methods:
      • TB Assessment Form signed by a medical professional - CA School Tuberculosis (TB) Form OR
      • Proof of Negative TB from a medical professional OR
      • ​​Cleared TB Chest X-Ray
  • Acceptable Proof of Covid Vaccination or Negative Covid Status can be shown by:
    • Copy of Valid Covid Vaccination Card  OR
    • Copy of Digital Covid Vaccination via https://myvaccinerecord.cdph.ca.gov/ OR
    • for unvaccinated individuals, a Negative PCR test that was taken within 48 hours of volunteer date (test locations attached)
  • Proof of Valid Photo Identification

Additional forms needed if you would like to volunteer as a chaperone and/or driver:
  • CA LiveScan Application - can be taken to any UPS store, and processed for a cost ($30-60) 
    • If you have already had the LiveScan processed, please send a copy with the ATI number completed by the Live Scan Operator.
    • You can also check your status at https://applicantstatus.doj.ca.gov/ using said ATI number.
  • District Driver's Form
  • Proof of Valid Driver's License
  • Current Proof of Car Insurance showing: 
    • Minimum Liability limitations of Bodily Liability - $100,000/$300,000
    • Property Damage - $25,000
    • Coverage Dates
    • Name of Drivers 
    • Make/Model/Year of Car (should be same as stated on Volunteer Driver Form)
    • ID CARDS ARE NOT ACCEPTED ​
